Clinical approach, risk stratification, screening algorithms, and shared decision-making. Covers cisgender women, cisgender men, and transgender individuals with HIV. The most directly applicable guideline for NYC metro providers.

International Landmark

IANS Consensus Guidelines for Anal Cancer Screening

The first comprehensive international screening guidelines. Risk-stratified initiation: age 35 for MSM/TW with HIV, age 45 for other PWH and MSM/TW without HIV, 10 yrs post-transplant for SOT recipients, within 1 year of vulvar precancer/cancer diagnosis.

Supports multiple screening strategies: anal cytology, hrHPV testing (including HPV16 genotyping), and co-testing. Recommends DARE at all screening visits.

Vaccination is the primary prevention strategy for HPV-related anal cancer. These ACIP recommendations apply to all providers counseling patients on HPV vaccination.

ACIP recommendations: routine at 11-12, catch-up through 26, shared decision-making 27-45. Gardasil 9 prevents anal cancer from HPV types 16, 18, 31, 33, 45, 52, and 58.

The study that changed the standard of care. Essential reading for every coalition member, and the foundation for every guideline and policy change in this toolkit.

ANCHOR Study: Treatment of Anal HSIL to Prevent Anal Cancer

Phase 3, 25-site RCT. Treatment of anal HSIL in PWH reduced progression to cancer by 57%. This study's dissemination fundamentally changed the screening and billing landscape.

After median 2+ year follow-up: 9 cancers in treatment group vs. 21 in monitoring group. Study visits concluded March 2024.
